You cannot. No note format built for clinical work exists, no assessment, no diagnosis field, nothing of that kind, and none of it is on a roadmap. What a Session carries is an administrative trail your staff can read, for lines like rescheduled or invoice resent.
Neither. Eligibility is not checked, claims are not submitted and no superbill is produced. You invoice a client for what they pay you directly, and since an invoice is built from line items, a sliding scale rate is simply what you put on the line.
